Change of Address
Families moving to another address within Natick must present documents to the Natick Public Schools that prove their new residency in Natick. The Natick Public Schools has the right to verify residential information through the Registrars Office at any time. The school will not make any changes to the student's file or school transportation until the Change of Address Form is completed and the documents required below are presented to the Registrar's Office.

If you are unable to provide proof of residency with any form of documentation as listed above, you will need to complete a Residency Affidavit.

∙ Caregiver Authorization Affidavit (MGL Chapter 511 of the Acts of 2008 as amended by Chapter 20 1F)
